چکیده

I n the current issue of European Respiratory Journal, in a German multicentre study, SCHULZ et al. [1] report a high frequency of sleep apnoea in patients with congestive heart failure (CHF). They also state that obstructive sleep apnoea (OSA) is more common in patients with heart failure than previously reported. Moving into the field of cardiology, the authors suggest that sleep studies should be routinely performed in patients with CHF and left ventricular dysfunction.

Increased long-term mortality in heart failure due to sleep apnoea is not yet proven.

Previous small-scale studies of the effect of sleep-disordered breathing (SDB) on prognosis in congestive heart failure (CHF) are either lacking or conflicting. The aim of this study was to assess the impact of the presence and type of SDB on mortality in a patient group with severe CHF referred to a specialised heart failure centre. Out of 78 patients ((mean +/- SD) 53 +/- 9 yrs, left ventricu...

Sleep apnoea and the heart.

Sleep apnoea is associated with significant daytime functioning impairment and marked cardiovascular morbidities, leading to a significant increase in mortality. Sympathetic activation, oxidative stress and systemic inflammation have been shown to be the main intermediary mechanisms associated with sleep apnoea and intermittent hypoxia. There are now convincing data regarding the association be...

Sleep is more than a break from life.

Man spends one-third of his life in sleep. Sleep is not a passive state but rather an active process that if disturbed on a regular basis can even constitute a threat to man’s health. Sleep medicine is a fairly new discipline and until 1980 there were only few scientific publications on the theme. Obstructive sleep apnoea syndrome was first described in 1976 by Guilleminault. Since then it has ...

Treatment of obstructive and central sleep apnoea in heart failure: practical options

Sleep apnoea, both central and obstructive disordered breathing, commonly occurs in patients with heart failure. Obstructive sleep apnoea occurs both in systolic and diastolic heart failure and is best treated with nasal positive airway pressure devices. Central sleep apnoea occurs primarily in systolic heart failure and therapeutic options are

Automated screening for sleep apnoea in the home environment

We present a simple method for detection of sleep apnoea in the home environment. The method identifies episodes of disordered breathing and provides an estimated apnoea hyponoea index (AHI) by recording and automatic analysis of nighttime ECG and oximetry recordings. At the heart of the device is a pattern recognition system that identifies episodes of apnoea by analysing the heart variability...
